Research Focus

My clinical research is dedicated to finding meaningful interventions for patients with behavioural disturbances due to dementia. These interventions range from pharmacologic ways of compensating for neurotransmitter imbalances caused by the dementia to non-pharmacologic means, such as specialized Day Programs or dance. Against a backdrop of No Known Cure, clinical research in dementia is a deep frontier for neuroscientists. The most Frequently Asked Question in my Frontotemporal Dementia Clinic is, “Dr., how long will this dementia go on? What do we expect to happen over time?” My research goals have arisen from my best response thus far, that there are four phases to this behaviourally-disturbed type of dementia, beginning with a questionably symptomatic phase, a definitely abnormal behavioural and personality change that intensifies and almost always requires pharmacotherapy, then a third stage, during which the patients symptoms subside and become withdrawn, amotivated, and intermittently agitated. The fourth stage for this dementia resembles the final stages of all neurodegenerative dementias.

My research is an attempt to identify what causes that crucial shift from stage 2 to stage 3. Thus far, I cannot link that shift to a critical degree of structural atrophy.

My efforts have utilized in vivo radioligand PET imaging and functional MRI methods, as opposed to post-mortem tissue staining that is difficult to connect to the shift in behavioural disturbances we observe over the course of the illness. The long-range goal is to identify a clear target to inform pharmacotherapy.

Currently, I am most intrigued by an apparent subclinical response of cerebral cortex in FTD to memantine, which is hypothesized to have neuroprotective properties. The placebo-controlled crossover study to follow-up on results from the initial open-label FDG-PET study will launch later this year.

The nature of behavioural disturbances in dementia belies brain-behaviour correlations taught from lesion studies: by the time patients are diagnosed, the ravages of FTD and Alzheimer's disease have struck in several places, not just within one frontosubcortical circuit. Patients paradoxically manifest both apathy and hyperactive disinhibition during the first half of the illness.

As one of the investigators in the multidisciplinary Frontotemporal Dementia Workgroup, I am dedicated to advancing innovative and meaningful research in FTD by optimizing collaboration among investigators, facilitating patient and family participation in research, and providing a collegial, educational forum for investigators, clinicians and their trainees.
